Development of an Individualized Health Care Plan (IHCP) occurs when the student has a specialized medical need or diagnosis which must be assessed, managed and monitored in school such as a life threatening allergy, diabetes, or seizure disorder. The nurse will meet with the parent/guardian to review medical information, physician orders and recommendations, current nursing research and evidenced based practice. The IHCP is then reviewed and signed by the parent and physician as needed, and shared with appropriate school staff to ensure optimal integration of the students health needs into the school setting. An ongoing exchange of information between the school nurse, parent and health care provider is essential to supporting the students success in school.
