Health Records
An
individual health record (paper and computerized) is maintained for
each student throughtout his/her school career. The record
conatins medical information submitted to the nurse, such as
immunization records and health inventory forms. Screenings and
visits to the health office are also recorded in the student's health
record.
Parents/guardians are asked to communicate student health
concerns and medical reports to the school nurse. Access to this
record is restricted to school health office staff and other school
staff on a need-to-know basis only, as determined by the school nurse.
In
accordance with the law, health records will be maintained in the health office till the student turns 23 y.o.
Several forms assist School Health Services staff to provide care to your child. Please feel free to print the forms and fill out prior to the first day of school and bring to the school nurse.
